Wet Age-related Macular Degeneration – Referral Pathway
High Street Optometrist | Upon examination if the patient presents with symptoms of Wet Age-related Macular Degeneration (wAMD) as per NICE NG82 please refer on the new Cinapsis Eyes Electronic Referral System (EeRS) using the AMD referral proforma. Refer to either Nottingham University Hospital (NUH) or Sherwood Forest Hospitals (SFH) based on the patient’s choice of provider. ONLY if your practice does not have access to Cinapsis EeRS then please refer via email on the details below. Do not refer the patient to their GP nor the community triage services if the patient has suspected wAMD. If the patient does not meet the criteria for onward referral as per NICE NG82 but requires onward care please do refer to the Nottingham and Nottinghamshire community triage services EMMS Healthcare PLC (City patients only) or HealthHarmonie. |
Self-referral | If the patient has been seen previously in the Macular Clinic and discharged the patient can self-refer via telephone. |

All patients will be triaged within 1 working day (Mon – Fri) and patients will be contacted by telephone within 48 hours of triage to arrange an appointment. The outcome of the initial triage will be shared on Cinapsis EeRS and the patient will be booked into clinic in accordance with the outcome. |
