You can now make referrals into the Community Low Vision Service for patients registered with a GP in the Nottingham area via their website at: www.emmshealthcare.co.uk
Referrals they receive for patients registered with a GP outside of the Nottingham area, will be passed onto the appropriate provider.
Please note: – the community service is only commissioned to provide a limited range of near and distance aids. Also if a patient you are referring requires sight impairment registration, you will need to refer them to the HES via their GP.
